How All-on-4 Works
Traditional implants need solid bone under each one, and people who have been without teeth for a while often do not have it. All-on-4 solves that by angling the two rear implants so they anchor in the denser bone toward the front of the jaw. Four implants, sometimes six, then support a complete arch of teeth. For most patients that means no bone grafting, fewer surgeries, and a shorter road to a finished result.
Dr. Acuña has advanced training in the All-on-4 technique and plans every case on a 3D cone beam CT scan with a computer guided surgical guide, so the implants go exactly where the plan says.

There is no gap, no denture in a drawer, no waiting. The day your implants go in is the same day you walk out with teeth, a full arch that Dr. Tostado already designed before you ever sat in the chair.
What Happens, Step by Step
- Complimentary consultation. You meet both doctors. A 3D scan, photographs and an honest conversation about your goals. You will hear whether All-on-4 is right for you, what the alternatives are, and what it will cost, in writing.
- Design before surgery. Dr. Tostado designs your new teeth first, so the surgical plan serves the smile rather than the other way around. You will see the design before anything is placed.
- Surgery day. Sedation is available and most All-on-4 patients choose it. Extractions, implant placement and your temporary teeth all happen in one visit.
- Healing. Three to six months on a softer diet while the implants fuse to bone. You have teeth the whole time.
- Final teeth. Dr. Tostado fits your permanent arch, adjusts your bite and refines the look in the lab until it is right.

Many patients come to us after a consultation at a large implant center. We welcome that. Bring your treatment plan and your scan, and Dr. Acuña and Dr. Tostado will give you a second opinion at no charge. Sometimes the answer is that the plan you have is good. Sometimes it is that you need fewer implants than you were told, or that you can keep teeth you were told to lose. Either way, you will know.
Paying for All-on-4
This is a major investment and we treat it that way. You get one written fee that covers the plan, with no surprises added later. Most patients use Cherry or CareCredit to spread the cost over monthly payments, and you can check pre-qualification before your visit. If you have PPO insurance, we will file for any benefit your plan allows.

Frequently Asked Questions
Is All-on-4 the same as dentures?
No. A denture rests on your gums and comes out. All-on-4 teeth are fixed to implants and stay in your mouth. They are cleaned like natural teeth and do not move when you eat or speak.
Can I get All-on-4 if I was told I have too little bone for implants?
Often, yes. The technique was developed for exactly that situation. The angled rear implants use the bone you still have toward the front of the jaw, which is why many patients avoid grafting. Your 3D scan will show whether that applies to you.
How long does All-on-4 take?
You leave surgery day with a full set of temporary teeth. Final teeth are usually placed three to six months later, once the implants have fused to the bone.
Will I be asleep for the surgery?
You can be. We offer oral conscious sedation and IV sedation, and most full arch patients choose one of them. Dr. Acuña will go over the options with you at your consultation.
What does All-on-4 cost in San Antonio?
The fee depends on whether it is one arch or two, whether extractions are needed, and the type of final teeth. You will receive the complete fee in writing at your consultation. Financing through Cherry and CareCredit is available.
Why should both a surgeon and a prosthodontist be involved?
The implants are the foundation and the teeth are what you live with every day. Dr. Acuña makes the foundation right. Dr. Tostado, a prosthodontist, makes the teeth fit your bite and your face and builds them in our own lab. Having both in one office means the plan is one plan.
